Current Openings
Registered Nurse (RN)
Full-Time / Part-TimeProvide skilled nursing care, symptom management, and patient assessments for hospice patients in their homes and care facilities. Work collaboratively with the interdisciplinary team to develop and implement individualized care plans.
Requirements:
- •Current Texas RN license
- •Minimum 1 year clinical nursing experience
- •Hospice or home health experience preferred
- •Valid driver's license and reliable transportation
- •Compassionate, patient-centered approach to care
Licensed Vocational Nurse (LVN)
Full-Time / Part-TimeSupport registered nurses in delivering compassionate hospice care, including medication administration, patient monitoring, and family education under the supervision of an RN or physician.
Requirements:
- •Current Texas LVN license
- •Home health or hospice experience preferred
- •Strong communication and interpersonal skills
- •Valid driver's license and reliable transportation
Certified Nursing Aide (CNA)
Full-Time / Part-TimeProvide personal care, hygiene assistance, and compassionate support to hospice patients in their homes. Help patients maintain comfort and dignity in their daily lives.
Requirements:
- •Current Texas CNA certification
- •Experience in home health or hospice preferred
- •Genuine compassion for patients and families
- •Valid driver's license and reliable transportation
Medical Social Worker (MSW)
Full-TimeProvide psychosocial assessments, emotional support, and care coordination for patients and families. Connect families with community resources and support them through the challenges of serious illness.
Requirements:
- •Master's degree in Social Work (MSW)
- •Current Texas LMSW or LCSW license
- •Experience in hospice, palliative care, or healthcare setting preferred
- •Strong assessment and counseling skills
Chaplain / Spiritual Care Coordinator
Part-Time / PRNProvide spiritual care and emotional support to patients and families of all faiths and backgrounds. Offer presence, comfort, and guidance during one of life's most profound transitions.
Requirements:
- •Theological education or equivalent training
- •Clinical Pastoral Education (CPE) preferred
- •Ability to provide non-denominational spiritual care
- •Experience in hospice or healthcare setting preferred
Bereavement Coordinator
Part-TimeProvide grief support and bereavement counseling to families before and after the loss of their loved one. Develop and implement bereavement programs in accordance with Medicare guidelines.
Requirements:
- •Degree in counseling, social work, or related field
- •Experience in grief counseling or bereavement support
- •Compassionate, empathetic communication style
